PET ADOPTION CONTRACT

The Adopter agrees to adopt the following animal (hereinafter called "Animal") from Siberian/ Shepherd Safe Haven Inc Animal Rescue, referred to as SSH .

As Adopter, I agree to the following:

To allow a representative of SSH to visit my premises to insure the terms of this agreement have been kept;

The Animal will not be kept strictly outdoors;

The Animal will NEVER be kept on a chain;

The Animal will be provided with adequate fresh food and water, clean, dry shelter when outside, and daily exercise;

To obey all applicable laws governing control, welfare and custody of animals;

To provide all medical care and treatment needed by the Animal including but not limited to: yearly medical checkups and vaccines, and preventative heartworm medication;

To adopt the Animal only as a personal pet/companion and not as a gift, working animal, or guard dog;

The Animal is not be be sold or given away for any reason. If it becomes necessary to find the Animal another home, SSH will be contacted and the Animal shall be returned to them;

I understand that I am assuming all financial responsibility for any illness that becomes apparent after the signing of this contract. I also understand that any illness that has been diagnosed and is being treated at the time of the adoption will be brought to my attention. I will receive instructions and medication, if necessary, for treating my new dog. As of the signing of this contract any illness, new or existing, becomes my full responsibility. _______

1. I understand I am responsible for keeping track of and informing myself of my dog’s medical history that has been provided to me._______

2. If I am no longer able or willing to care for the dog(s) I will contact SSH first and give SSH the option to obtain dog(s) without charge. The dog(s) may not be sold, given away, abandoned, or turned into animal control. I agree to give SSH 2 weeks to arrange for pickup or delivery of dog back to SSH in this event.________

3. I understand SSH cannot guarantee the age, health, breed, temperament of any dog. I further understand that this dog was rescued and the history of it’s past is unknown, this dog may have existing issues mentally and/or physically and I agree to meet and work through those issues patiently and lovingly. _________

4. I understand if I violate this agreement the dog may be seized, or I as the new owner may be held financially responsible for any and all legal/court, shelter/animal control or other fees._____

5. If at any time in the future I have any questions, concerns or complaints I agree to contact SSH directly._______

6. SSH reserves the right to come and visit/check on my adopted dog without notice at anytime.

I agree to abide by the adoption conditions and realize that I am committed to this animal for the rest of his/her life. I have read and fully understand that SSH may repossess this animal at any time if the adoption conditions are violated or the animal is mistreated.
